I would add one eventually. I would at least put a full mattress cover on it. There are tons out there. Almost all say water proof but most are soft and water resistant. No one wants to sleep on crunchy plastic anyways. I would just throw it in a full mattress protector. I would also buy a mattress pad. It’s just the 1 inch of cotton that goes on like a fitted sheet.between the two if there’s an accident it shouldn’t get to the bed. And this is what i mean by a pad. The pad won’t change the feel of the bed (might slightly cool down memory foam) but is mainly protection. Here’s a general idea of what it looks like. Everyone says waterproof but it’s mainly water resistant. So you see what it looks like. The only one that would alter the bed would be something like a feather bed topper (they tend to be 3 inches) just so you know what to avoid.
